iPad™ Sweepstakes Rules:
1. How to enter and entry eligibility: Enter by visiting www.metro-dentalcare.com/iPad and filling out sweepstakes form. Metro Dentalcare employees and their immediate family members are not eligible. Minnesota and Wisconsin residents only. Only one entry per person. Must be 18 years or older. Entries must be received by March 28, 2011 at 11:59 PM ET.
2. No purchase necessary and purchase of any product or service will not increase your odds of winning.
3. Prizes and odds of winning: One iPad™ will be given away to a randomly chosen winner. Value of the iPad is $500. The odds of winning depend on the number of entries received. Winner is responsible for any tax reporting obligations.
4. The winner will be contacted by a Metro Dentalcare representative between March 29-April 5th. Metro Dentalcare will make three attempts to contact the winner via e-mail and phone. If the winner cannot be reached after three attempts, the winner will forfeit the prize. The winner’s acceptance of the prize gives Metro Dentalcare permission to publicize the winner’s name on a winner’s list.
5. Metro Dentalcare may cancel or modify this sweepstakes at any time. Entrants agree to accept the prize “as is.” No returns or exchanges will be accepted. Metro Dentalcare is not responsible for any problems with your entry, including technological failures. By participating, entrants agree that Metro Dentalcare and its related organizations shall have no liability for any damages of any kind that may result from the use of the prize.
