Eligibility

1. This promotion is valid for hygiene (routine cleaning and check up) appointments only, during the months of April and May, Monday through Friday between the hours of 10:00 a.m. and 2:00 p.m. 

2. Patients must keep and show up for the appointment. Patients must make their reward selection on an entry slip upon checking in for their appointment. 

3. Names will be drawn twice a month per participating practice and your name will remain in the pool throughout the entire promotion.

4. Metro Dentalcare will contact all winners. Odds of winning will depend on number of entrants. 

5. Open to current or new patients. If winner is under the age of 18, the prize will go to the parent or legal guardian. 

6. Metro Dentalcare employees and their immediate family members are not eligible to participate. 

*The $50.00 credit will be taken off after insurance payment, if applicable. No cash back.
