RULES AND ELIGIBILITY REQUIREMENTS
WHO MAY PLAY. This contest is open to Minnesota and Wisconsin resident, who are18 years or older at the time of entry. Full and part-time Metro Dentalcare and American Dental Partners’ employees, its advertising agencies and promotional companies involved in any promotion, as well as family and household members of the same, are not eligible.
PRIZE. The prize winner will receive a $500 credit toward a Family Block Appointment or future dental services, if applicable. The treatment will be conducted at a Metro Dentalcare clinic of the winner’s choice. A Family Block Appointment constitutes a block of time during which members of a family receive preventative dental treatment, also known as hygiene visits (exam, teeth cleaning and/or x-rays). Depending on the size of selected dental clinic and number of staff available, family members can be seen simultaneously or in time increments. Metro Dentalcare will make every effort to accommodate families of all sizes. Larger families may need larger blocks of time and/or multiple visits.
If the winner’s preventative treatment (hygiene visits) is covered in-part or entirely by insurance, the remaining or full credit can be applied toward a future dental service, such as fillings, crowns or whitening. The credit can be used by the winner, winner’s spouse and dependents. The credit must be used by October 31, 2010. No cash back.
New patients: New patients must complete a new patient hygiene visit. The $500 will first be applied toward the new patient visit, and any remaining portion can be applied toward future dental service.
CONTEST AND ENTRY DEADLINES. The contest begins on July 27, 2009, and ends on October 31, 2009. One winner will be drawn per month during the months of August, September and October 2009. A random drawing will occur on or around the last day of each month. A total of three (3) winners will receive the contest prize - $500 toward a Family Block Appointment.
NO PURCHASE IS NECESSARY TO ENTER OR TO WIN. Only one Entry Form may be submitted by each eligible participant. All entries are subject to verification. Metro Dentalcare shall not be responsible for entries that are late, lost or destroyed or for any printing or administrative or computer error of any kind in connection with this contest.
CHANCES OF WINNING A PRIZE. The chances of winning will depend on the total number of correct entry forms submitted and received by the entry deadline as described above. Winners will be selected in a random drawing from all eligible entrants by employees or representatives of Metro Dentalcare.
PRIVACY AND PUBLICITY. By participating in the contest, each contestant acknowledges and agrees that Metro Dentalcare and American Dental Partners, Inc. have the right to reproduce, broadcast and publicize his/her name, likeness, photograph, city of residence, and if applicable, the fact that he/she is a winner of the contest, and all matters incidental to thereto, for publicity purposes and without further compensation.
GENERAL. These rules are subject to change without notice and reason including, without limitation, the need to comply with any applicable law or regulation. Metro Dentalcare and American Dental Partners, Inc. reserve the right to cancel or suspend this contest at any time for any reason. Winners are solely responsible for reporting and payment of any taxes on prizes. Winners will be responsible for all necessary travel and other expenses incurred in connection with accepting the treatment.
